• All work must comply with the National Electrical Code (NEC) and relevant Federal, State, and Local laws.

• Materials must be standard products of manufacturers and meet specified requirements.

• Workmanship must be to journeyman standards.

• A one-year warranty on all materials and workmanship is required.

• Maximum effort to divert C&D debris from landfills through recycling and reuse.

• Location: Bldg. SD002, 3401 Boland Dr, South Bend, IN.

• Coordination Schedule: Required at least 2 weeks prior to project start.

• Monthly Status Reports: Due on or before the 25th of each month.

• Contractor must coordinate with the Facility Operations Specialist (FOS) and Contracting Officer's Representative (COR).

• Personnel require identification for facility access; a list of contractor/subcontractor personnel must be submitted 5 working days in advance.

• Adherence to safety practices as required by OSHA and the Contractor's Insurance Provider.

• Contractor is responsible for all permits, licenses, and fees.

• Site maintenance includes daily cleanup and proper disposal of waste off-site.

• Smoking is prohibited except in designated areas.

Statement of Work (SOW) / Performance Work Statement (PWS)

This SOW outlines the requirements for a project to mill down and repave Parking Vehicle (POV) lots #1 and #2, replace sidewalk and curb, and paint light pole bollards at the BELTON USARC in Kansas City, MO.

• Removal: Contractor shall remove existing parking blocks, mill approximately 1.5"-2" of asphalt pavement (approx. 130,000 sq. ft.) from POV lots #1 and #2, remove and dispose of approximately 2,340 lineal feet of concrete curb and gutter, approximately 350 lineal feet of 5ft wide concrete sidewalk, and a concrete pad (approx. 70" x 90").

• New Work:

  • Apply tack coat (approx. 130,000 sq. ft.) using emulsified asphalt.
  • Apply Hot Mix Asphalt (HMA) job mix formula (approx. 130,000 sq. ft.) to an average depth of 1.5"-2", meeting State DOT regulations.
  • Restripe POV parking lots, matching existing markings (e.g., no parking hash lines, handicap markings, stall lines, centerline, turning lane, double line, fire lane).
  • Paint twelve (12) light pole bollards with two coats of safety yellow paint.
  • Reset parking blocks.
  • Provide and install signage (ADA, traffic, fire lane).
  • Replace damaged curb and gutter (approx. 2,340 lineal feet).
  • Install new concrete sidewalk sections (approx. 350 lineal ft x 5’ wide) with 4000 psi concrete.
  • Install a new 30 ft wide concrete entrance on the northwest end of POV Lot #1 with 4500 psi reinforced concrete.
  • Place a new concrete slab (approx. 70" x 90") north of the USARC with 4000 psi concrete.

• Materials: Aggregate base course must meet State DOT regulations. All materials shall be new and of the latest type/model.

• Asphalt compaction: 95% of standard proctor density.

• Aggregate base compaction: at least 95% of laboratory maximum dry density.

• All work to be done to journeyman standards.

• Materials and workmanship warranted for at least one (1) year from final acceptance.

• Location: 15303 Andrews Rd. Kansas City, MO.

• Coordination Schedule: Required two weeks prior to project start.

• Monthly Status Reports: Due on or before the 25th of each month.

• Waste Disposal: Contractor shall track and report all construction and demolition debris using the 88th RD Waste Disposal Form. Maximum effort for recycling and reuse is required.

• Safety: Comply with OSHA and insurance provider requirements. Control dust during milling operations.

• Scheduling: Notify COR five (5) working days prior to construction start. Coordinate work to avoid interference with military operations. Obtain approval for work outside normal hours.

• Access & Security: Provide lists of personnel requiring access, including DOB and ID numbers. All personnel must display badges/IDs. Vehicles must be parked in designated areas.

• Submittals: Coordination Schedule, Paint type/colors, Monthly Status Reports, Warranties, Waste Disposal Form, O&M manuals, cut sheets, product samples.

• Utilities: Temporary water available from Government; Contractor to make connections and provide backflow prevention. Contractor personnel may use existing sanitary facilities.

• Environmental: Comply with all Federal, State, and Local laws regarding debris disposal. Control dust and avoid creating nuisances.
