This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It requires offerors to self-certify their status as an "Indian Economic Enterprise" in accordance with the Buy Indian Act (25 U.S.C. 47) and HHSAR 326.601. This certification is a prerequisite for responding to solicitations, requests for information (RFIs), or resulting contracts that are set aside or awarded under the Buy Indian Act.

• Solicitation/Sources Sought/RFI #: Field to identify the specific procurement.
• Project Title: Field for the project's name.
• Date: Field for the date of completion.
• Representation: A checkbox section for the offeror to indicate whether they represent that they do or do not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Name of Federally Recognized Tribal Entity: Field for the name of the relevant tribal entity.
• Name of the 51% owner of IEE Firm: Field to identify the majority owner.
• Business Name: Field for the offeror's business name.
• Unique Entity Identifier (UEI) Number: Field for the offeror's UEI.
• Certifying Signature: Signature line for the authorized representative.
• Print Name: Printed name of the certifying individual.

Bidders responding to opportunities under the Buy Indian Act must complete and submit this form to self-certify their eligibility. Failure to meet the definition of an "Indian Economic Enterprise" throughout the solicitation, award, and performance periods may result in disqualification or contract termination. Offerors must also be registered in the System of Award Management (SAM). Providing false information is a violation of law and subject to penalties.

Statement of Work (SOW) / Performance Work Statement (PWS)

To maintain continuity of critical and timely telecommunication services for the Kayenta Service Unit, supporting its mission to improve the physical, mental, social, and spiritual health of American Indians and Alaska Natives.

• Provide voice and data services over standard circuit-switched and digital circuit systems.

• Deliver services for Kayenta Health Center, Inscription House Health Center, and Dennehotso Health Station.

• Include minor hardware and software changes/installation, and maintenance for telecommunication services.

• Offer dedicated high-bandwidth voice and data communication services (T1, DSL, Point-to-Point, MSDN, etc.).

• Provide digital channel service, direct inward dialing, exchange telephone service, local and long-distance calling plans, access services, calling services, line extensions, routing, security, and data path services.

• Offer technical support for communication equipment, fiber optic, and microwave/radio equipment maintenance and repair.

• Ensure continuity and resiliency in telecommunication services for voice and data networks.

• Provide monthly invoicing.

• Maintain availability for technical support and billing inquiries.

• Conduct hardware or software assessments, replacement, and repair as needed.

• Period of Performance: June 01, 2026 – May 31, 2027 (base year), with a 4-year contract option.

• Place of Performance: Three locations within Arizona:

  • Kayenta Health Center, Kayenta, Arizona
  • Inscription House Health Center, Tonalea, Arizona
  • Dennehotso Health Station, Dennehotso, Arizona

• Government-furnished property includes network cabling, network access, internet and telephony services, and building electrical power at the specified locations.

• Contractor must coordinate with the Division of Information Technology for project planning, schedules, and access to U.S. Government property, adhering to security requirements for sensitive areas.
