The solicitation is a Request for Quote (RFQ) to establish a multiple award Blanket Purchase Agreement (BPA) contract vehicle. Statement of Work (SOW)

This Statement of Work (SOW) outlines the requirements for providing Bath & Shower items to the Federal Emergency Management Agency (FEMA) for Transportable Temporary Housing Units (TTHUs). These items are intended to provide accommodations for disaster survivors who require specific accessibility features.

The Contractor shall furnish all materials, supplies, equipment, and related resources necessary to provide Bath & Shower items as specified in this SOW. This includes selecting items that meet specified requirements, complying with fire and safety standards, and delivering them to designated FEMA locations.

• Bath & Shower Items: Specific items include various sizes of barrier-free showers with beveled thresholds, roll-in showers, accessory packages, T-shaped water stopper kits, ADA-compliant safety/grab bars (12", 18", 24", 48"), and padded shower seats. Shipping is also included.

• Compliance: Items must meet American National Standard A117.1, Uniform Federal Accessibility Standards (UFAS), Americans with Disabilities Act (ADA), and 16 Code of Federal Regulations (CFR) Chapter II Subpart Consumer Product Safety Act.

• Substitutions: Any proposed substitute products must be submitted in writing to the Contracting Officer (CO) or Contracting Officer's Representative (COR) for approval before shipment, detailing how the substitute meets the SOW specifications.

• Condition: All items must be new, unused, and delivered in original manufacturer/factory packaging.

• Ordering Period: Five (5) years from award.

• Delivery: Typically to FEMA Manufactured Housing Storage Site (MHSS) in Cumberland, MD, but the contractor must verify the specific location in each Request for Call Order Proposal (RFCOP). Delivery is required within 30 calendar days from the date of the order.

• Delivery Coordination: Contractors must coordinate delivery windows with the Point of Contact (POC) at the delivery location, considering site limitations.

• Documentation: Deliveries must be accompanied by an itemized Bill of Lading (BOL). A spreadsheet detailing delivery totals must be provided to the COR and delivery location POC within one business day of delivery.

• Inspection & Acceptance: Government inspection and acceptance will be in accordance with FAR 52.212-4(b).

• Damaged Items: The Contractor is responsible for repairing or replacing damaged items within 72 hours of notification, at their expense for shipping replacement items and returning damaged ones.

This document is Attachment 4, a pricing table for RFQ No. 70FB8026Q00000022. It is used by bidders to input unit prices for various shower and accessory items across multiple ordering periods.

• Items: Includes multi-piece barrier-free showers (various sizes), roll-in showers, accessory packages, water stopper kits, and ADA-compliant safety grab bars (various lengths).
• Estimated Quantity: Provided for each item and should not be changed by the bidder.
• Pricing Columns: Unit Price and Total Price for Base Year, Option Year 1, Option Year 2, Option Year 3, and Option Year 4.
• Totals: Auto-calculating fields for total price per item, total for each ordering period, and the total BPA price (Base plus Options).
• Instructions: Bidders must input unit prices; totals will auto-calculate. Do not alter estimated quantities.

This pricing table is critical for bidders to calculate their proposed costs for the opportunity. Bidders must accurately input their unit prices for each item and ordering period as instructed. The estimated quantities are fixed, and the table will automatically compute the total costs, which will be a key factor in proposal evaluation.
