USCG Base Cape Cod Backflow repair, replacement and retest

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide all personnel, equipment, tools, materials, supervision, and services necessary to repair, replace, and retest six (6) backflow preventer devices at USCG Base Cape Cod (BCC).

• Repair and retest one (1) Watts, model # LF009, 3” domestic backflow device at building 3154.

• Replace and retest one (1) Ames, model # Colt 400, 6” fire backflow device at building 3160.

• Replace and retest one (1) Watts, model # 909, 6” fire backflow device at building 3170.

• Repair and retest one (1) Watts, model # 009M2QT, 1” domestic backflow device at building 3426.

• Replace and retest one (1) Watts, model # 009M2QTRP, 2” domestic backflow device at building 5203.

• Repair and retest one (1) FEBCO, model # 805, 4” fire backflow device at building 5205.

• Testing and maintenance procedures are specified for devices smaller than and greater than 4 inches, including check valve operation, air bleeding, leak inspection, cleaning, and reporting deficiencies.

• Submit documentation verifying compliance for each device, including date of completion, certification, location, model, serial number, USCG ID#, manufacturer, size, device type, and Public Water Supplier information.

• Report any damaged or deficient devices with a proposal for repairs.

• Work to be performed at USCG Base Cape Cod (BCC) on Joint Base Cape Cod, Buzzards Bay, MA.

• Government working hours are 7:00 am to 3:30 pm, Monday through Friday. Work may be coordinated after hours or on weekends/holidays with COR authorization.

• Submittals: Include contact information, Certificate of Insurance, Base Access Request Form, Safety Data Sheets, Closeout Submittal, Safety Plan, and Qualification Statement with quote or as specified.

• Qualifications: Technicians and on-site supervisors must hold valid Massachusetts Backflow Prevention Device Tester Certificates. Employees must be trained and experienced.

• Security: Personnel require base access, which involves completing a Base Access Form, providing personal information for background checks, and obtaining Coast Guard Security Identification (ID) cards. Vehicle operators must possess valid licenses.

• Coordination: Schedule all work with the COR and BCC personnel, considering Coast Guard mission requirements.

• Safety: Develop and submit a written Safety Plan prior to commencing work.

• Appearance: Contractor personnel must wear uniforms and nametags, and display security ID cards at all times on base.
