This solicitation is for non-emergent wheelchair and stretcher transportation services required by the Lake City VA Medical Center and its associated Community-Based Outpatient Clinics (CBOCs) within the North Florida/South Georgia Veterans Health System. The contractor will provide all necessary vehicles, personnel, management, supplies, and equipment to offer these services 24 hours a day, seven days a week, including weekends and holidays.

• Provision of non-emergent wheelchair and stretcher transportation services.

• Transport of patient luggage, medical records, and medication is permitted.

• Contractor personnel must be courteous, professional, and possess valid driver's licenses and required certifications.

• Personnel are subject to background investigations (Special Agreement Checks - SAC).

• Contractor must provide a tuberculosis (TB) exposure control policy and ensure personnel TB testing.

• Vehicles must meet all applicable federal, state, and local regulations, including safety and comfort standards.

• Vehicles must be clean and maintained in good repair.

• Contractor must utilize VetRide or VA-approved transportation software for all transports.

• Invoices must be submitted electronically via HCFA 1500 format within 30 days of service.

• Loaded mileage is billable; unloaded (deadhead) miles are not.

• Response times for unscheduled transports are up to 30 minutes.

• Contractor must maintain a Quality Control Plan (QCP).

• Contractor must comply with COVID-19 and other infectious disease protocols, including providing necessary PPE.

• Period of Performance: Base period from Date of Award through 07/31/2027, with four 12-month option periods extending through 07/31/2031.

• Estimated Value: $19 Million (for the base period, as indicated on SF 1449).

• Evaluation will be based on Technical factors (Performance Plan, QCP, Vehicles/Equipment, Staffing, VetRide utilization) and Price.

• Offerors must be certified as an SDVOSB by the SBA and listed in the SBA certification database at the time of offer and award.

• Compliance with Limitations on Subcontracting (LOS) for services is required (50% to non-SDVOSB firms).

• The solicitation incorporates numerous FAR and VAAR clauses related to compliance, insurance, security, and payment procedures.

• Service Contract Act wage determinations are provided for various Florida and Georgia counties.
