Statement of Work (SOW) / Performance Work Statement (PWS)

To provide performance-based grounds maintenance services to keep the Western Oregon Service Unit (WOSU) campus safe, clean, accessible, and hazard-free for patients, staff, and visitors.

• Provide all labor, supervision, equipment, tools, materials, supplies, transportation, consumables, and incidentals for complete grounds maintenance.

• Services cover all exterior areas of the WOSU facilities, including buildings, parking lots, sidewalks, landscape areas, retention pond areas, garden beds, and grounds within the property line (approx. 3.71 acres).

• Major work areas include routine service visits and periodic grounds/landscape maintenance.

• Routine Service: Inspect/maintain retention pond, collect/remove trash and debris, sweep/clean paved areas, remove weeds/vegetation from lawn/garden/paved areas, remove cobwebs/debris from exterior structures, haul away all collected debris.

• Periodic Maintenance: Mow/edge lawns, edge around trees/shrubs/buildings, trim bushes/trees, water bushes/trees (as coordinated), fertilize, aerate/power rake lawn (annually), remove fallen leaves (fall season), prune trees/bushes (annually).

• Chemical Application: Use approved products, follow label directions, prevent drift/runoff, provide Safety Data Sheets. Coordinate fertilizer/tree spike applications.

• Snow/Ice Removal: Provide as-needed removal when directed, aiming for substantial completion before 7:00 a.m. on business days. Clear sidewalks, steps, stairways, ADA ramps, and parking lots; apply ice melt.

• Performance Standards: Adherence to service schedule, campus cleanliness, clear/safe access routes, functional retention pond, effective vegetation control, proper chemical control, timely snow/ice response, and accurate reporting.

• Place: Western Oregon Service Unit (WOSU), 3750 Chemawa Rd. NE, Salem, OR 97305.

• Period: Base year (12 months) with four one-year option periods. Start date to be identified in award/Notice to Proceed.

• Government-Furnished: Access to exterior work areas and a water source.

• Contractor-Furnished: All personnel, supervision, equipment, tools, herbicides, fertilizers, ice melt, consumables, PPE, and other necessary materials.

• Pre-performance Meeting: Required within two weeks after Notice to Proceed; services cannot commence until held.

• Hours of Operation: Routine services Monday-Friday, 7:00 a.m. - 5:00 p.m. (excluding Federal holidays), unless otherwise authorized.

• Service Schedule: Minimum 12 hours of routine service per week, adjustable based on weather/operations but coordinated in advance.

• Staffing: Sufficient staff and supervision to complete work safely and on schedule. At least one responsible lead person must be onsite.

• Safety: Comply with facility procedures, safety/security requirements, and applicable regulations. Wear appropriate PPE. Work must not disrupt patient care.

• Equipment: Vacuum/blower use restricted near clinic entrances, building entrances, or HVAC intakes to avoid affecting indoor air quality.

• Reporting: Submit a written service report with each invoice detailing services performed, hours, issues, chemicals used, etc.

• Submittals: Proposed service schedule, staffing approach, equipment list, and chemical product information required before work begins. Updates needed for new chemicals or approach changes.

• Invoicing: Submit electronically via IPP, weekly or monthly. Include service report with each invoice.
