Statement of Work (SOW) / Performance Work Statement (PWS)

To provide non-personal laboratory support services to assist the Portland Area Indian Health Service, Yakama Service Unit in delivering direct patient medical care in a clinical laboratory setting. The services support the overall health care status of the eligible patient population.

• Provide one (1) Medical Laboratory Scientist and one (1) Medical Technician (Phlebotomy) in a moderate complexity outpatient clinical laboratory setting.

• Services include diagnostic testing, phlebotomy, specimen collection and handling, quality control, documentation in the electronic health record, patient flow, and coordination with health care providers.

• Work is to be performed during clinic operating hours, not to exceed forty (40) hours per week.

• Major work areas include chemistry, hematology, urinalysis, serology, and microbiology testing, as well as phlebotomy.

• Services must be timely, accurate, safe, and compliant.

• Performance will be monitored for quality and appropriateness, including review of service delivery, compliance with clinical and regulatory requirements, and identification of performance deficiencies.

• Quality control, calibration, infection control, biosafety, and regulatory compliance are required.

• Place of Performance: Yakama Service Unit, 401 Buster Road, Toppenish, WA 98948.

• Period of Performance: One (1) six-month base period and one (1) six-month option period. Each period not to exceed 120 workdays.

• Staffing: One Medical Laboratory Scientist and one Medical Technician (Phlebotomy).

• Qualifications: Specific certifications (Medical Laboratory Scientist/Technician/Technologist/CLS, CPR) and experience (at least 1 year for both roles) are required. Personnel must meet Yakama Service Unit credentialing and background requirements.

• Compliance: Adherence to CLIA, COLA, OSHA, HIPAA, Privacy Act, Indian Health Service credentialing/background requirements, and applicable state/national standards.

• Security: Compliance with privacy, confidentiality, facility access, credentialing, background, and health information security requirements, including HIPAA and Privacy Act.

• Government-Furnished Equipment (GFE): The Government will provide facility space, equipment, supplies, and systems necessary for laboratory operations.

• Post-Award Documentation: Required within ten (10) days of award, including Certification of Immunization, Declaration of Employment, Certification of Education and Insurance, HIPAA Agreement, and Addendum to Declaration for Federal Employment.

• Professional Liability Coverage: Minimum of $1,000,000 per occurrence.

• Taxes: Contractor responsible for applicable Federal, State, and local taxes.
