• Penalties: False or misleading information is a violation of law, punishable under 18 U.S.C. 1001, and false claims are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
• Representation Section: Requires the offeror to check a box indicating whether they do or do not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Information Fields: Includes spaces for the Name of Federally Recognized Tribal Entity, Name of the 51% owner of IEE Firm, Business Name, Certifying Signature, Print Name, and UEI Number.

Statement of Work (SOW)

To provide secure, on-site shredding services for sensitive documents for the Yakama Service Unit, ensuring confidentiality and integrity of destroyed materials.

• Lease of locked shred bins and provision of secure on-site shredding.

• Monthly shredding services and on-demand, one-time shredding services upon customer request.

• Shredding must meet or exceed cross-cut standards recommended by the National Security Agency (NSA)/Central Security Service (CSS).

• Provide service reports, manifests, or certificates of destruction upon request.

• Notify the Facility Manager at least 48 hours in advance of scheduled service or anticipated interruption.

• (8) 64-gallon wheeled bins with document slot

• (3) 32-gallon wheeled bins with document slot

• (2) consoles with locking mechanisms

• All storage units shall be keyed alike, with a copy of the key provided for each bin and console.

• Paper shredded to particles measuring 1 millimeter by 5 millimeters or smaller (cross-cut).

• Includes shredding of all paper-based media and similar materials.

• Contractor personnel will have intermittent access and will be escorted by IHS employees while on-site.

• No background investigation is required for contractor employees.
