• Completed Survey Form: A rating scale (1-5) for various performance aspects, with specific definitions for each rating level (Unsatisfactory, Marginal, Neutral, Satisfactory, Very Good, Exceptional). A narrative explanation is required for ratings of 1 or 2.
• Performance Areas: The questionnaire covers 20 specific areas of performance, including management, technical abilities, risk identification, responsiveness to changes, attitude, quality control, schedule adherence, safety, and overall assessment.
• Remarks Section: A dedicated space for detailed explanations, especially for low ratings or specific questions.

Solicitation / RFP / RFQ

This is a presolicitation notice for construction services to correct failing plumbing conditions in Building 1 at the Central Alabama Veterans Health Care System. It is not a Request for Proposal (RFP), and contractors should await the official RFP posting.

• Removal of all dead leg piping sections.

• Re-application of insulation to missing segments of hot water (HW) piping.

• Installation of one chlorination injection station per floor.

• Increasing water heater temperature to 140°F and installing mixing valves where absent.

• Extending DHW recirculation (DHWR) piping to within 6 feet of fixtures and installing small mixing valves at sinks/lavatories.

• Replacement of DHW recirculation pumps for larger flow rates.

• Installation of a chiller system (air-cooled or with cooling tower/condenser water system), heat exchanger, and chilled water pumps to cool domestic water below 68°F.

• Insulation of all existing and new chilled water (CW) piping.

• This acquisition is a 100% Set Aside for Certified Service-Disabled Veteran-Owned Small Businesses (SDVOSBs).

• Firms must be certified and listed in the SBA certification database (veterans.certify.sba.gov/) and registered in SAM.gov at the time of submission.

• Compliance with VAAR Clause 852.219-73 and 852.219-75 is required.
