Submission & Evaluation

Offerors must also submit Exhibit D (Contractor Evaluation Form Construction Safety) and Exhibit E (Surety Form), and provide past performance information for up to five similar projects within the last five years using Exhibit A and B. A bid bond is required with the proposal, and performance and payment bonds will be required from the awardee.
