Scope of Work

Contract Details
This effort will result in multiple Blanket Purchase Agreements (BPAs) with an anticipated 5-year ordering period. The authorized Ceiling Limit for the agreement is $7,000,000.00, with individual BPA orders estimated at $10,000.00. Pricing will be based on Centers for Medicare and Medicaid Services (CMS) Healthcare Common Procedure Coding System (HCPCS) DMEPOS A-Code and L-Code fee schedules, minus any offered discounts. The solicitation is open for two years from its initial issue date, with specific quote due dates communicated via SAM.gov for "on-ramp" opportunities to add new vendors.
Eligibility & Set-Aside
This acquisition is unrestricted. The primary NAICS Code is 339113 (Surgical Appliance and Supplies Manufacturing) with a size standard of 500 employees. While unrestricted, the evaluation will consider Service-Disabled Veteran-Owned Small Business (SDVOSB) and Veteran-Owned Small Business (VOSB) status. Vendors must employ full-time, ABC or BOC certified Orthotists at each service location, and facilities must be ABC or BOC accredited. A minimum 90-day warranty is required for all orthotic appliances.
