This document is Attachment B, the Past Performance Questionnaire (PPQ). It is to be used for past performance evaluations when a CPARS Evaluation is not available. Offerors must submit this form for projects not covered in the CPARS database, completed by an owner or owner's representative not affiliated with the Offeror's firm.

• Contractor Information: Firm Name, CAGE Code, Address, DUNS Number, Phone, Email, Point of Contact.
• Work Performed As: Prime Contractor, Subcontractor, Joint Venture, or Other.
• Contract Information: Contract Number, Task Order Number, Contract Type, Contract Title, Location, Award Date, Completion Dates, Original and Final Contract Price.
• Project Description: Complexity, relevance to the current opportunity, and details of similar work.
• Client Information: Name, Title, Phone, Email, and Client's role in the project.
• Performance Ratings: A detailed questionnaire for the client to rate the contractor's performance across Quality, Schedule/Timeliness, Customer Satisfaction, Management/Personnel/Labor, Cost/Financial Management, Safety/Security, and General aspects using adjective ratings (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Applicable).
• Remarks: Space for narrative comments on strengths, weaknesses, deficiencies, or overall performance.

This PPQ is a critical component for demonstrating past performance. Bidders must ensure they provide accurate client contact information and instruct their clients on how to complete and submit the questionnaire. The ratings and narrative comments provided by the client will be used by the government to evaluate the Offeror's past performance risk. Offerors should explain any past performance resulting in significant cost and/or time growth. The completed PPQ must be submitted with the proposal, unless a CPARS evaluation is available. Questionnaires may be submitted directly to the Government points of contact (Mr. Andrew Sprys or Ms. Alicia Evans) if requested by the client. Offerors will receive email confirmation upon receipt of successfully submitted PPQs.

This document contains conceptual site plans and axonometric views related to the John Day Lock and Dam Transportation and Site Access project. It is provided for reference only, as drawings are subject to change.

• Sheet C-102 (Conceptual Site Plan): Outlines proposed new visitor access roads, parking areas (contractor and guard booth), a new Auxiliary Monitoring Station (AMS) building, a new security gate and fence, a water storage tank for fire protection, and a new turbine storage building. It also indicates areas for contractor staging and a future drain field.

Solicitation / RFP / RFQ

This solicitation is for a two-phase design-build project at the John Day Dam in Rufus, Sherman County, Oregon. The project involves designing and constructing a new Access Control Point (ACP), reconfiguring existing roads to accommodate oversize load vehicles, and enhancing security on the Oregon entrance side of the dam. The scope includes a security guard booth with an inspection canopy, an Auxiliary Monitoring Station (AMS) building, final design analyses, design documentation report, final design plans and specifications, construction execution, and a turn-key AMS and ACP.

• Design and construction of a new Access Control Point (ACP).

• Reconfiguration of existing roads and design of new roadways and parking lots.
