Offerors are instructed to distribute this questionnaire to their references and ensure it is completed and returned to the designated government email addresses by a specified deadline.

• Offeror Information: Fields for the Name and Address of the Offeror being evaluated.
• Contract Details: Space to record information about the contract being referenced.
• Evaluator Information: Fields for the Evaluator's Full Name, Position, Company/Organization, Address, Phone Number, and Email Address.
• Past Performance Survey: A series of questions rated on a scale of Outstanding (O) to Unacceptable (U), covering aspects such as compliance with requirements, timeliness, labor and safety standards, management effectiveness, personnel practices, quality control, flexibility, reliability, responsiveness, and handling of issues. Specific questions address cure notices, willingness to re-award, customer satisfaction, notification of problems, invoice accuracy, professionalism, promptness, and meeting expectations.
• Remarks Section: An area for evaluators to provide additional detailed comments.
• Submission Instructions: Clear directives for Offerors on how to distribute the questionnaire and for Evaluators on how to complete and return it directly to the government, not back to the Offeror.
